When all required elements are configured you can also use vRealize Orchestrator workflow as item in the catalog. To do this you need to have XaaS Architect role.

When you adding the new XaaS you can create a Custom Form
After adding to Service and setting Entitlement you are able to request new XaaS workflow from Catalog.
